In many agencies, CQI appears on the calendar once a month. Reports are reviewed. Trends are discussed. Action items are assigned. Then everyone returns to operations. That is not discipline. That is an event. Continuous Quality Improvement was never designed to function as a meeting. It was designed to function as a leadership habit.
